Payment Information:
At the time of your visit, we will collect any appropriate co-pays as dictated by your insurance company. For self-pay patients, please inquire about our self-pay fee schedule when you schedule your appointment. Generally fees are collected on the day of your visit though we will arrange for a payment schedule if necessary. For your convenience, our office accepts Visa, Mastercard, checks, and cash.
Please call us at (510) 527-8865 by noon on the day prior to your scheduled appointment to notify us of any changes or cancellations. If prior notification is not given, a missed appointment fee of $50 may be applied.
